The Apollo 204 Review Board was established by the Administrator, National Aeronautics and Space Administration, under the authority of NASA Management Instruction 8621.1, dated April 14, 1966. Dr. Robert C. Seamens, Jr., Deputy Administrator, NASA, in Memoranda dated January 28, 1967, and February 3, 1967, appointed Board Members, defined Board responsibilities and amplified oral instructions to the Board Chairman, Dr. Floyd L. Thompson. Twenty one task panels were formed to provide administrative and technical assistance to the Board. The Board's Final Report was issued April 5, 1967, and the Board was officially dissolved October 7, 1967.

Created or accumulated by components of the Apollo 204 Review Board to investigate the disastrous Apollo 204 spacecraft module accident of January 27, 1967 (which claimed the lives of three NASA astronauts, Lt. Col. Virgil I. Grissom, Lt. Col. Edward H. White, and Lt. Cmdr. Roger B. Chaffee), these records document the formation and organization, powers and responsibilities, and functions and activities of the Review Board. A principal element of the Board's formal review of the circumstances surrounding the accident was the creation of twenty-one task panels manned by experts in the respective fields. The reports of these panels provided the main sources of information from which the Board formulated its findings and recommendations. These were subsequently embodied in its final report to the Administrator. The records primarily comprise extensive technical documentation including reports, studies, analyses, evaluations, summaries, synopses, diagrams, charts, and graphs provided by the task panels for the Review Board; printed copies (4 vols.) of testimony of NASA officials before House and Senate oversight committee hearings; and general subject correspondence documenting NASA's rapid response to the Apollo 204 tragedy. Attention was focused primarily upon oxygen test histories of the crew compartment systems, and arcing and shorting problems experienced during those tests. All environments were reviewed for rationale used in original deviation, substantiation by ground and flight vehicle tests, and margins imposed when implemented into component level tests. The other environments were examined as possibly being germaine to the cause of the accident. These reviews included the vibration, heating, and humidity environments.

It was tasked to display significant events as they occurred with a precise time tag. Time histories of all continuous or semi-continuous recorded parameters, correlation of parameter variations and events were recorded as well as an interpretation of the results of said analysis. Where pertinent, normal expected variations were compared with those actually obtained. Panel 3 served as a separate panel from January 30, 1967, through February 23, 1967. It was dissolved on February 23, 1967, and merged with Panel 18, Integration Analysis.

Task Panel 4 developed plans and procedures for progressive disassembly of the Apollo 012 spacecraft for purposes of inspection and failure analysis. Disassemby was configured to proceed on a step-by-step basis, in a manner to obtain the maximum amount of information. Contents of testing plans were considered. Disassembly plans included both the cockpit and the area outside the pressure hull. Means for cataloging documentary information within the spacecraft and for the display of removed items were a part of the plans and procedures.

Other areas of review included egress augmentation and basic cabin atmosphere concept (one versus two gas).

This series consists of autopsy reports and post-mortem photographs of the astronauts killed in the Apollo 204 accident. In addition, there are photographs of the post-fire condition of the spacecraft, which includes views of the astronauts still inside, and others of them still in their suits after removal from the command module. Lastly, there are some records concerning the injuries sustained by ground support personnel in the fire and rescue attempts. Task Panel 11 was assigned the responsibility of providing a summary of medical facts together with appropriate medical analysis which would be of interest to the investigation. Examples given included the cause of death, pathological evidence of overpressure, and any other areas that might be of technical value in determining the cause of the accident or in establishing corrective action.
